What is Loathsome Mire actually for in PoE 2?
Loathsome Mire is one of those places that can throw you off the first time you step in. It looks like an endgame area, but it doesn't behave like a normal farming map. It was added with Path of Exile 2 update 0.5.0, Return of the Ancients, and it sits inside the new Delirium setup rather than the Runes of Aldur crafting loop. If you're sorting through PoE2 Items and wondering why people care about this odd little zone, the answer is simple: it exists for a very specific amulet reward, not for piles of random loot.

Why does the area feel so empty when players enter it?
You're not imagining it. Loathsome Mire has special rules that make it feel strange compared with regular maps. Players don't gain or lose experience there, monsters don't drop items, and the area doesn't drop league tablets. So if you run around expecting currency, rares, tablets, or normal Delirium clutter, you'll probably think something's broken. It isn't. The whole point is to reach the reward tied to the encounter. That's also why old habits, like full-clearing every corner, can waste your time here.

How do you find Loathsome Mire during Delirium maps?
The practical part starts around The Withered Willow, the Delirium hub added in 0.5.0. Maps near it always contain Delirium and help build your Delirium Atlas Passive Tree. Once you break a Delirium Mirror, watch the progress bar at the bottom of the screen. That bar shows how far you've pushed into the fog, with the map boss sitting at the far end. Special Delirium events appear along that bar, and community guides point to Loathsome Mire as one of the events you need to reach before the fog disappears. In plain terms, keep moving forward. Don't wander off for side packs unless your build is fast enough to get away with it.

What drops from Loathsome Mire, and why do players want it?
The confirmed reward is two new amulet base types. These bases can grant two instilled notables, which is a big deal for builds that need extra passive power without spending points on the tree. The catch is nasty, though. One version loses a prefix slot, while the other loses a suffix slot. Community talk commonly calls them Twisted Amulet and Distorted Amulet, but the official notes only confirm the two-base setup and the prefix-or-suffix penalty. That lost affix matters. A caster might hate losing a suffix if it blocks key cast speed or attributes. A tankier setup might care more about prefixes for Life or Energy Shield. It's not an automatic upgrade. You've got to judge the notables against the missing craft space.

What should you do before farming it seriously?
First, update your loot filter. Several players have said they missed the amulet because it wasn't highlighted, which is a miserable way to waste a rare-looking run. Second, treat the zone like a targeted reward room, not a loot map. Third, bring enough survival for Delirium pressure, especially if your build relies on Energy Shield recharge or big leech hits, since 0.5.0 changed both areas quite a bit. If you trade often, it's worth checking rolls carefully before selling or crafting, and players who prefer to buy cheap Path of Exile2 Items should still learn what the two-notable combinations are worth, because a bad-looking amulet can be build-defining for the right character.
